Blended Capitation model

Garden Medical Clinic has adopted a new model of care called the Blended Capitation Model. Through this model, our goal is to continue providing you with excellent, personalized care tailored to your health needs-while enhancing how we deliver that care. 

• The Blended Capitation Model was designed over a five-year period by Alberta Health and the Alberta Medical Association, utilizing best-practice research. 
• Garden Medical Clinic will receive a set payment to provide health services to you. We will receive this payment regardless of the number of visits we provide to you - this de-incentivizes the over-provision of health services (like having you come in for your test results when we can instead tell you over the phone). 
• As an accountability measure to ensure that your access to the clinic is maintained, the clinic will be "negated" when you go to see another clinic for an in-basket service. For example, let's say that you have a health concern but the next available appointment for your doctor is three weeks away. If you decide to go to a different walk-in clinic, Garden Medical Clinic will be penalized for the full cost of that health service that you received elsewhere.  

• At Garden Medical Clinic, patients have access to primary care providers and our full interdisciplinary team across two convenient locations. Our team-based model improves access to care, helping you get the appointment you need-when you need it. 
• We offer care in innovative ways including: in-clinic visits, telephone appointments, and email correspondence. 
• Our online booking system, Medeo, provides you with the ability to book your own appointments online and email with your physician. 
• Research indicates that when a patient continually sees the same health care provider, their health outcomes improve because of the continuity of care.  

• Contact us first for your medical needs. This improves continuity of care and helps you avoid long wait times at walk in clinics. 
• If you feel you may need care at another clinic for specialized concerns, please speak with your Primary Care Provider at Garden Medical Clinic first. 
• Update us with any changes in your health and contact information.
