The Alberta Health Care Insurance Plan pays for many of the services you receive from your doctor. However, the government health plan does not cover uninsured third-party services. These include but are not limited to travel consultations, insurance documentation, and legal forms. These services require the time and expertise of your primary care provider and staff. As a result, you will be billed a fee to cover these costs. Payment will be expected when you receive the service.
Following CPSA guidelines, primary care providers complete uninsured third-party services within 30 days of the appointment date.
All forms must be submitted to our Forms Department for registration before they can be reviewed. Completion of the form is at the discretion of your primary care provider.
Once the form has been completed, our Forms Department will contact you to provide information about any applicable fees.
Completed forms can be picked up Monday - Friday from
09:00 a.m. to 12:00 a.m. and 1:00 p.m. to 4:30 p.m.
